SUPPORT CONNECTION AND COORDINATION

10. SUPPORT COORDINATION

Support coordination is a capacity building support which helps you to understand and use an NDIS plan and to pursue your goals. It connects you with NDIS providers, community, mainstream and other government services. It also helps you to build your confidence and coordinate your support.

Our core mission and values are all client focused. The care and support offered to all our clients is our number one focus. Our primary aim is to give the choice and control to the participant and we feel the way to achieve this is by working together to get the best results possible.

There are 3 levels of support coordination:-

1. Support Connection

This aims to build your ability to connect with informal, community and funded support enabling you to maximise your plan and pursue your goals.

2. Support coordination

coordination of supports. This support will assist you to build the skills you need to understand and use your plan. A support coordinator will work with you to ensure a combination of different types of support are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and to feel more included in your community

3. Specialist support coordination

This  is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. A specialist Support Coordinator will assist you to manage challenges in your support environment and ensure consistent delivery of a first class service.

You may also wish to focus on a specific goal within your plan, such as those related to finding a suitable home or help with assisted living. You can use your support coordination funding to work with a support coordinator who has the, knowledge and experience to assist with your specific needs.

Your funding is based on your specific needs and on what is reasonable and necessary to pursue your goals, in addition to any support provided by your friends, family and any other community and government services.

Each participant has a Capacity Building Budget which will include, where reasonable and necessary support coordination. This will be a fixed amount for a support coordinator to help you use your plan.

If your plan states the level of support coordination funded, you canonly purchase this level of support coordination. If it doesn’t state it, you can choose to purchase the level of whatever suits your needs.
